Submit an Incident Report
Montgomery College students have certain rights and responsibilities and are expected to abide by the Student Code of Conduct (PDF, 
) . You may report violations of the Student Code of Conduct using the forms below.
There are four different incident reports which can be filed. If this is an emergency or you need immediate help, contact the Office of Public Safety and Emergency Management or 911 from any College phone.
1. Academic Integrity Report new window: Use this form to report incidents such as academic dishonesty, cheating on assignments
or exams, forgery, submitting materials that are not the student's own. This list
is not exhaustive. A full list of violations can be found in Section VIII: Academic
Dishonesty of the Student Code of Conduct (PDF, ) .
2. Standards of Conduct Report (Non-Academic) new window: Use this form to report incidents such a disruption or obstruction of the education
process of the College; physical or psychological abuse; intimidation; misuse of computing
resources; gambling; possessing, using, selling, or distributing any type of drugs
for illegal reasons; defacing, destroying, or damaging property; theft; unauthorized
use of facilities or commission of any offense prohibited by local, state or federal

3. Behaviors of Concern Report new window (Behavioral Intervention Team): Use this form to report incidents such as: bizarre
thoughts, depression, excessive anxiety, mood swings, inappropriate display of emotions,
or general concerns about a student. This form is not to report misconduct or violations of the Student Code of Conduct. Behaviors of Concern
reports will be addressed by the Behavioral Intervention Team.
4. Sexual Misconduct Report (Title IX) new window: Use this form to report incidents of sexual misconduct, including sexual harassment,
sexual assault, domestic violence, dating violence, and stalking. This form is for
individuals seeking support and information after experiencing sexual misconduct,
third party witnesses, anonymous reporters and College employees fulfilling their
mandated reporting duty to share information disclosed to them by a student. This
form does not initiate a formal investigation, but will prompt a response from the
Title IX Coordinator.
Behavioral Intervention Team
The mission of the Montgomery College Behavioral Intervention Team (BIT) is to maintain a healthy, safe, and secure environment, free of intimidation and harm, by systematically addressing behaviors of concern. The BIT is comprised of staff, faculty, and administrators who work at that campus.
Step 1. An incident report is filed to the BIT Chair and/or Dean of Student Affairs
Step 2. The BIT Chair and/or Dean of Student Affairs determine whether to proceed
as a BIT case or other
Step 3. If a BIT case is opened, the campus-based team is made privy to the information
Step 4. A background investigation begins and the campus team is convened
Step 5. The BIT reviews information to determine the threat level, if any
Step 6. If a threat exists, the BIT recommends an intervention to the BIT Chair and/or
Dean of Student Affairs
Step 7. Intervention takes place and team follows up as appropriate
